It DOES NOT replace rheumatological care! How to use the Luna – step by step 1 Define your goal and choose the mode Before sleep → Sleep. Daytime stress → Relax. Meditation, focus → Medit. Headache, chronic pain → Relief. For an acute migraine attack, the Relief mode can bring noticeable relief within 20 minutes for some users, but the true effect builds with twice-daily use over 4 weeks. 2 Prepare the earplug Moisten the electrode surface of the left earplug with contact gel (or water spray if the gel has run out). Without a moist contact the stimulation will not be effective! 3 Position it in your left ear The electrodes should contact the cymba concha and cavum concha areas of the ear – this is the auricular branch of the vagus nerve. Poor positioning may make the stimulation uncomfortable or ineffective. Follow the diagrams in the user manual! 4 Start and gradually adjust the intensity Plug the earplug cable into the device, press and hold the power button for 3 seconds. Start from ZERO intensity, and slowly increase to the level you feel is “strong but not painful.” 5 20 minutes – sync your breathing The Relax and Medit modes use an intermittent rhythm (4 s on / 4 s off). Breathe with the device: exhale during the pulse, inhale during the pause. The Sleep and Relief modes provide continuous stimulation – here just close your eyes and let it work. 6 Maintain the routine for at least 4 weeks The effect of taVNS is cumulative. Don’t expect miracles after one or two sessions – but daily 1–2 sessions for 2–4 weeks can already bring noticeable changes. To reduce the number of headache days, the literature recommends at least 4 weeks of twice-daily use. Why might vagus nerve stimulation work for headaches? Several mechanisms may play a role in migraine and tension-type headache: activation of the trigeminovascular system, central sensitization, neuroinflammation and imbalance of the autonomic nervous system. The auricular branch of the vagus nerve that Luna stimulates is directly connected to these pathways – via the nucleus tractus solitarii it can influence the function of the periaqueductal gray (PAG), the locus coeruleus and thalamocortical networks. fMRI studies (Zhang et al., 2020) showed that after 4 weeks of taVNS treatment in migraine patients not only did monthly headache days and pain intensity decrease, but measurable modulation of thalamocortical connections was observed. This structural neuromodulation explains why the effect is cumulative. Moist contact is necessary for stable electrical conduction between the metal electrode and the skin of the ear. Conductive gel provides a more stable and less drying contact than plain water. Regular users should consider purchasing replacement gel. Cleaning and maintenance After each use wipe the electrodes with a soft, dry cloth. Once a week perform a deeper disinfection with an alcohol wipe. NEVER wash the main unit under running water! The silicone fitting can be cleaned with warm, soapy water. Frequently asked questions about the Luna Which is a better choice for me: Luna or Vita? If your main concern is sleep, stress management or daily relaxation, the Vita with 3 modes is a good choice. If, however, headache, migraine, chronic pain are also in the picture, or you want one device to cover multiple issues – Luna is recommended because it includes the fourth Relief mode. Additionally, the Luna’s battery lasts twice as long and enables 20+ sessions from a single charge. How long until effects appear for migraine? During an acute migraine episode some users report pain relief after a 20-minute Relief session, but this varies significantly between individuals. The preventive effect – a reduction in monthly headache days – typically requires at least 4 weeks of twice-daily use according to the literature. Straube et al. (2015) used a 3-month study period. The effect is cumulative: it does not develop without regular use. Can I use it alongside my migraine medications? The Luna works non-invasively and independently of drugs, so it does not have direct drug interactions. However, DO NOT change or stop prophylactic or acute migraine medications prescribed by your neurologist because you bought the device. Luna is an adjunct tool, not a replacement for specialist care. Consult your treating neurologist if you want to use it in combination. Why is the earplug placed in the left ear? More than 65% of clinical trials used left-sided stimulation – this is the standard protocol in the literature. The right vagus nerve has stronger connections to the cardiac conduction system, so for safety reasons non-invasive taVNS treatments prefer the left side. A right-side earplug can be purchased separately, but use only one at a time! What does the Relief mode feel like? Will it worsen the headache? The Relief mode provides continuous but gentle stimulation – initially you may feel light tingling or pricking that quickly softens into a gentle vibration. If you try it during a migraine attack and experience any worsening (increasing pain, nausea, light sensitivity), stop the treatment immediately. In some patients in the acute phase, taVNS may not be effective – which is why starting treatment between attacks as a preventive approach is recommended. Can I use it if I have a pacemaker or epilepsy? NO. Pacemaker, ICD, cochlear implant, deep brain stimulator or any implanted electronic device are absolute contraindications. Epilepsy is also a contraindication – interestingly, VNS was originally developed to treat epilepsy (as an implanted device under medical supervision), but unsupervised at-home taVNS is NOT recommended for patients with epilepsy. It should not be used during pregnancy due to lack of clinically validated data. How many times per day and for how long can it be used? Based on manufacturer protocols and clinical trials, 1–2 times daily for 20 minutes per session is recommended. For preventive headache use, twice daily (morning + evening) for 4 weeks is the minimum for effect development. The literature does not define a strict maximum safe use, but more than 3–4 sessions per day is not recommended – "more = better" does not apply here.
