Guide How to choose an incontinence treatment device?

Choosing an incontinence treatment device doesn't start with which is cheapest or which looks best. The question is: what type of problem do you have, what muscle function needs improvement, and what level of feedback do you want from the treatment?

A simple electrostimulator may be suitable for mild to moderate problems. You may need a biofeedback device if you want not only to treat the muscles but also to see, measure, and follow how they work. This is especially important if the pelvic floor is hard to activate, the exercise technique is uncertain, or you want a more conscious rehabilitation process.

First, identify the symptom

Not all incontinence is the same. Choosing the wrong type of device is like trying to replace sunscreen with a raincoat: it helps with something, just not what you need. The lists below present devices from simpler to more capable.

Which device level is right for you?

Devices differ not only in price. The main difference is whether they only provide stimulation or can also measure, give feedback, and document muscle activity.

Do you need a probe?

An incontinence treatment device is most effective when used with an intracavitary probe. This can be a vaginal or anal probe. The probe's role is to deliver the electrical impulse directly to the target muscle area, and in biofeedback treatments to sense muscle activity or squeeze pressure.

Some devices include the appropriate probe in the package. For others you must select a vaginal or anal probe separately. Always check this before buying!

Function Electrostimulation, EMG biofeedback or pressure-biofeedback?

Electrostimulation

In electrostimulation the device delivers mild electrical impulses that can help activate the pelvic floor muscles or sphincters. This is particularly useful when the muscles are weak, difficult to recruit, or the user cannot clearly feel how to engage them.

Electrostimulation works "passively," meaning you can relax during the treatment; you don't have to do anything actively.

EMG biofeedback

EMG biofeedback does not primarily stimulate but detects the muscle's electrical activity. The device gives feedback on whether the pelvic floor is actually working. This helps you learn correct muscle activation and makes progress more trackable.

Biofeedback devices (whether EMG or pressure-based) require your active participation! During treatment you will be given continuous "tasks" to voluntarily contract your pelvic muscles. The device senses your active muscle activity and provides feedback. In fact, the evoStim E adds stimulation assistance to a well-performed exercise. Combining pelvic floor exercises with electrical stimulation has been shown to be more effective than either exercise or stimulation alone.

Pressure-biofeedback

Pressure-biofeedback uses a balloon probe. The device measures how much pressure change occurs during a muscle contraction. This is mainly used to assess and train vaginal muscle strength or anal sphincter function.

The points described in the EMG section also apply here, with the difference that the device detects squeeze pressure rather than electrical activity. Otherwise everything works the same way.

When to expect results?

An incontinence treatment device is not a magic wand. You can't expect that it arrives on Monday, you use it once on Tuesday, and on Wednesday you wake up with Olympic-level pelvic floor muscles. Rehabilitation of the pelvic floor and sphincters requires regular, persistent treatment.

Generally, a minimum of 6–8 weeks of consistent use is needed to notice tangible improvement. For lasting results, a months-long daily routine is typically required. That's why choose a device you'll actually use, not one that will sit in a drawer.

Why is using only pads or diapers not enough?

Pads and diapers can be useful because they provide security and help avoid embarrassing situations. But it's important to know: these do not treat the cause. They absorb urine or stool but do not affect the condition of the pelvic floor muscles, do not improve sphincter function, and do not teach proper muscle control.

The goal of an incontinence treatment device is to support functional improvement, not concealment. In many cases a pad or diaper may be temporarily necessary, but if there is rehabilitatable muscle weakness, it's worth addressing the cause.

Tip Dr. Zátrok's advice

If you have mild to moderate symptoms and want a simple, good value home solution, start by combining pelvic floor exercises and electrostimulation (Kegel Toner, Biolito, evoStim UG, Sure Pro).

If you can't feel which muscle to contract or pelvic floor exercises seem ineffective, choose an evoStim E or evoStim P device.

If the issue is male post-prostate surgery leakage, don't buy a female device hoping "it will do". Choose a male device with an anal probe (Perfect PFE for men or evoStim UG).

If you're unsure, ask for advice before buying. A well-chosen device can help — a poorly chosen one will likely become an expensive drawer resident.

Contraindication Important safety information

Do not use intracavitary electrostimulation in the following cases, or only with medical approval if suspected:

  • Vaginal, bladder or rectal infection
  • Pregnancy
  • Genitourinary or colorectal (rectal–colon) tumor
  • Pacemaker or other implanted electronic device
  • Vesicoureteral reflux
  • Hypotonic (atonic) bladder

Warning Hygiene rule

An intracavitary probe is a personal hygiene device. A probe should be used by only one person — do not share it with others, even within the family. Family harmony tolerates many things, but this is better not to test.
